Next Steps in the Process
As the convening closed, the Next Steps in our process to complete the Health Disparities Framework and Diabetes Plan of Action were identified.
  • A final report for the Convening will be disseminated to all convening participants and those interested in this effort who could not attend by mid July. This, along with the PowerPoint presented at the convening will also be posted on the NEPO website.
  • The Expert Panel will finalize the draft framework and action plan over the summer, 2004.
  • The minigrant guidelines and RFP will also be completed over the summer and released at the October 2004 EPO Summit.
  • The final draft of the Racial and Ethnic Health Disparities Framework and Diabetes Action Plan will be presented at the October 2004 EPO Summit for final review and adoption.
  • The final plan will also be shared with the CMA Foundation board of directors and our funding partner, Pfizer, Inc. in late October.
  • We will announce "The Plan" through a press event in November 2004, Diabetes Awareness Month.
  • The adopted plan will be posted on the NEPO website with links made to partner organizations beginning in November.
  • The Health Disparities minigrant selection panel will be selected in September and October.
  • Health Disparities minigrants application will be received and evaluated in November and December 2004.
  • Grants will be announced at the end of December with funds made available to begin the projects in January 2005.
  • The Expert Panel will continue forward and oversee the implementation of the Diabetes Plan of Action. Prevention, Disease Management and Policy Subcommittees will be formed, chaired by a member of the Expert Panel. The Expert Panel will convene once by phone in November to plan its work for 2005.
