We would appreciate your taking a few moments to give
us feedback about the convening and ways to strengthen
our efforts as we move forward.
6. What can we do to help you
address disparities related to diabetes?
- Provide a resource directory.
- Further define strategies for preventing/treatment/policy.
- Develop or update the EPO directory or develop links.
- Strong policy agenda to increase access to diabetes
screening, treatment, and healthier foods for ethnic
communities. Policy agenda that supports healthier food
choices, how to shop for healthy foods, and healthier
cooking/food preparation.
- Get the information out on what is available already
out to practitioners so the community MD can get it
to the public.
- We can continue to partner with CMA.
- Create marketing campaign aimed at Disease Management
and Prevention - get out on the airways ASAP.
- Directory (with a short description of services and
calendar of events and meetings) of members, partners,
associates and state departments.
- Develop tools for physicians (and resources) to communicate
with patients/communities/policy makers.
- Keep meeting and working on your plan - set the model
at meetings - serve healthy foods and have physical
activity break - replace sodas with skim milk, water
and juice - replace refined carbohydrates with complex
carbohydrates.
- Communicate to me what is going on in other places,
not to reinvent the wheel.
- Keep sharing progress w/ DPCP so we can learn and
share with others.
- We are on the right track.
- Follow through in policy, prevention, and disease
management with clear goals and strategies.
- Keep the connections made today going to further develop
seedling ideas of today.
- Help fund my documentary on Disparities.
- Provide more forums of this nature to help those of
us that are community health advocates.
- Emphasize the importance of EPOs participating with
State and Local government health agencies who often
write grants or have funding to implement translation
research or community-wide education campaigns, etc.
