Disparities Action Plan - Type 2 Diabetes Focus (continued)
Disparities Framework
The framework adopted by NEPO is an attempt to move forward
HEALTH FOR ALL: California's Strategic Approach to Eliminating
Racial and Ethnic Health Disparities. The overarching
goals of this report have been incorporated into the NEPO
framework -
- Prevent the development of illness and injury by fostering
healthy behaviors, healthy community environments and
institutional support for good health outcomes.
- Reduce the severity of illness and injury by providing
high-quality medical care to all.
The NEPO framework identifies the Predisposing Factors
associated with a health condition and then the key Focus
Areas for possible intervention. Within each focus area,
opportunities for intervention are identified which become
the focal point for the development of goals and action
steps.

Expert Panel
An Expert Panel provides overall guidance and direction
for the development of the plan of action as well as expertise
in both addressing health disparities and type 2 diabetes.
The Expert Panel is comprised of experts in the field
of type 2 diabetes from American College of Clinical Endocrinologists
[AACE] along with ethnic physician leaders representing
the communities most at risk for Type 2 Diabetes and ethnic
physician leaders from health plans and public health
agencies and community-based organizations most effected
by the health disparities associated with type 2 diabetes.
